ATTACH STAIN STICKERS TO AREAS OF THE GOWN
Attach stain stickers to areas of the gown that may need special attention. Please use only the stickers provided. Other stickers may damage the gown. (There is no need to mark the hemline as it is the most commonly soiled area.)
PLACE GOWN IN THE BAG & BOX PROVIDED
Place gown (and any accessories) in the bag provided. Include left side of your pink Form, and payment for additional items in the bag with the twist tie provided. Place bag in box.
SHIPPING LABEL
Remove the bottom portion of the FedEx shipping label and place it on the back of the left side of the pink order form. Keep this for your records (your copy).
FOLD DOWN BOTH FLAPS
Fold down both flap “A”s. Then fold flap “B”s down over them. Close top of box with tape provided. NOTE: Tape provided needs moistening. You may seal the box with your own tape if preferred. Place the prepaid FedEx shipping label on the box top. Give to any FedEx driver, take to a FedEx Location near you.
